Providence Health Plan
We are in-network providers for Providence Health Plan at Portland Eye Care. Providence Health Plan offers both medical and vision coverage, however they are in the process of transferring the vision coverage for their members to Vision Service Plan (VSP). What this means for you as a Providence Health Plan member is that when you visit our office for routine vision care, your visit will be processed through VSP.
Since we are in-network providers with VSP, there will be no extra forms for you to fill out, and you’ll generally be covered for your eye exam with just a small copay, your contact lens evaluation with another copay, and you will usually then have an allowance to apply toward the purchase of contacts or eyeglasses. You’ll just need to let us know that you have Providence Health Plan with vision coverage through VSP and we’ll be able to verify your benefits. Generally VSP will require the name and date of birth of the primary member on the plan as well as the last four digits of their social security number. For Providence Health Plan members, they will also require your complete providence identification number and plan group number to release benefit information to our office. You can find your identification number and your plan group number on your Providence Health Plan card.
When you visit our office for medical eye care such as an evaluation for ocular effects of diabetes or an emergency visit for an ocular injury, you will be covered directly under your Providence Health Plan benefits and will only need to pay any co-pay or coinsurance applicable to your Providence Health Plan. With some plans, there may be a deductible that you have to meet before your Providence Health Plan will provide coverage.
